I was Diagnosed With Environmental Asthma. What Medication Will Help?

Nick: I was diagnosed with environmental asthma. The treatment has been going on for two years now with Flonase, Serevent, Flovent, theopylline and Singulair. The problem still persists and goes on for days with peak flow down to 190 and wheezing. Is there another approach or medication that is available for severe asthma attacks? I have been on prednisone for three years and haven’t had much luck with that either. I have also developed polyps and had surgery, but the polyps returned within 30 days. Is there any other help with this?
Dr. Kraemer: You describe a progressive problem with nasal polyps and only partially responsive severe asthma. Unfortunately, there is no simple fix. You are taking all of the approved medications, and even prednisone does not give a good result. You are probably already under the control of a specialist in your area. If you have tried all these medications, and were very rigorous about complying with the right doses, and they still don’t work, you need to talk with your doctor. Make sure that your diagnosis is clear, that you are using all of these treatments optimally, and that you are minimizing indoor allergens, workplace allergens, smoke, and your sinus and reflux problems.

If it still is not effective, then you may require a further consultation with a regional treatment center (like in San Diego, La Jolla, or Los Angeles). You could discuss with them if you would be a candidate for more aggressive sinus treatments, or using the newer nebulized corticosteroids (budesonide), or the newer infusions of humanized Anti-IgE antibodies (from Genentech), or any other investigational therapy for the most severe forms of asthma.
